  • Dual Checkpoint Immunotherapy

Dual checkpoint blockade with nivolumab plus ipilimumab is being tested across NSCLC, hepatocellular, biliary and neuroendocrine cancers, including PD-L1-low disease. Objective response and progression-free survival recur as endpoints, with anti-PD-L1 combinations and triplets challenging single strategies. Early detriment signals and variable benefit keep tumour-agnostic reimbursement and patient selection unresolved. The shared aim is to overcome PD-(L)1 resistance while preserving durable complete responses.

  • Renal Cancer Treatment Outcomes

First-line nivolumab plus ipilimumab is being judged against standard care in advanced renal cell carcinoma, with clinical scores and cost-effectiveness shaping selection. Papillary RCC remains less certain than clear-cell disease, while real-world esophageal squamous-cell data broaden the combination’s clinical scope. Outcomes and value vary across China and US models, leaving optimal eligibility and sequencing open. The work favors evidence-guided use rather than assuming combination therapy is universally worthwhile.

  • Immunotherapy Resistance in Cancer

Checkpoint immunotherapy is being examined through melanoma plasticity, brain metastasis outcomes and combination treatment of GIST. NRAS status, treatment discontinuation and entinostat or imatinib recur as possible modifiers of response. The studies do not establish one shared therapeutic direction, but they converge on resistance, tumour-state change and the limits of durable benefit. Clinical melanoma data sit alongside mechanistic and early combination evidence.

NEW DIRECTION

Ipilimumab becomes a continuously produced biologic rather than an intermittently administered checkpoint drug

The encapsulated cell-based “biologics factory” in immunocompetent mice treated ipilimumab as an antibody to be manufactured in situ, rather than as a drug delivered by repeated injection or infusion. The engineered allogeneic cell chassis produced ipilimumab among a diverse set of monoclonal antibodies, while an immunomodulatory alginate formulation reduced fibrosis and supported sustained antibody delivery. This places ipilimumab in a new therapeutic-delivery role: a single administration of antibody-producing cells could replace recurrent dosing and its associated peak–trough exposure and access problems 42826187Oct.
