Statin
Statins (formally classified as 3-hydroxy-3-methylglutaryl-CoA reductase inhibitors, or HMG-CoA reductase inhibitors) are a class of lipid-lowering agents that competitively inhibit HMG-CoA reductase, the rate-limiting enzyme in the mevalonate pathway responsible for endogenous cholesterol biosynthesis.
By blocking this enzymatic step, statins reduce hepatic cholesterol production and upregulate low-density lipoprotein (LDL) receptor expression on hepatocytes, thereby lowering circulating LDL cholesterol and reducing the risk of atherosclerotic cardiovascular disease. First approved for clinical use in the late 1980s, statins have become among the most widely prescribed drugs globally and are considered a cornerstone of primary and secondary cardiovascular prevention. Common members of this drug class include atorvastatin and simvastatin, which differ in their lipophilicity, potency, and metabolic profiles — properties that influence both their efficacy and their safety signatures across organ systems.
Beyond their canonical cholesterol-lowering mechanism, statins exhibit a range of pleiotropic effects — including anti-inflammatory, immunomodulatory, and antioxidant activities — that are only partially explained by reductions in LDL cholesterol or modulation of the mevalonate pathway. These additional biological actions have positioned statins as candidate therapies or adjuncts in a growing number of disease contexts beyond cardiovascular disease, including oncology, liver disease, neurological conditions, and inflammatory disorders. Their broad biological footprint, however, also underlies a spectrum of adverse effects and drug interactions that continue to be characterized in large-scale clinical and genetic studies.

Cardiovascular Prevention and Adherence
Cardiovascular prevention is moving toward sustained statin use, intensive treatment after ischemic stroke, and support for patients with diabetes or hypertension. Adherence trajectories and health-coach mobile interventions address the persistent gap between prescribing and long-term lipid control. Combination strategies include statins with dapagliflozin and immediate intensive statin therapy alongside dual antiplatelet treatment. Benefit remains balanced against uncommon adverse effects such as perioral neuropathic symptoms.

Statin Safety and Clinical Outcomes
Statin research is linking exposure to survival, disease progression, dementia, and pregnancy outcomes across several clinical populations. Observational analyses suggest lower progression and mortality in non-cirrhotic primary biliary cholangitis and possible survival associations in ALS. Other work tests whether combined statin and antihypertensive therapy alters dementia risk, while cohort data examine first-trimester exposure and pregnancy outcomes. The field remains dependent on propensity-adjusted and time-to-event analyses, with causality and subgroup safety still unresolved.

Cholesterol Signaling and Cancer
Cholesterol-dependent signaling is emerging as a target for tumor migration and treatment resistance. In melanoma, cholesterol-driven PIEZO1 activation supports amoeboid migration, while simvastatin may restore cisplatin sensitivity in cervical cancer through caveolin-1 and PI3K/AKT suppression. Boolean modeling and transcriptomics are being used to define how statin–ezetimibe combinations alter cholesterol synthesis and import. Human analyses additionally question whether statin exposure influences neuropathic pain, leaving cancer mechanisms and systemic effects at different stages of evidence.
