Gestational diabetes
Gestational diabetes mellitus (GDM) is glucose intolerance or diabetes first recognized during pregnancy.
Gestational diabetes mellitus (GDM) is glucose intolerance or diabetes first recognized during pregnancy. It develops when the normal pregnancy-related increase in insulin resistance—driven in part by placental and other hormonal changes—exceeds the pancreatic β-cell capacity to produce insulin, resulting in elevated maternal blood glucose. Risk is influenced by factors including maternal age, body mass index, obesity, previous GDM, and susceptibility to type 2 diabetes. GDM is clinically important because it is associated with adverse pregnancy outcomes, including hypertensive disorders such as pre-eclampsia, and identifies increased longer-term risk of metabolic disease in the mother. Diagnosis generally relies on antenatal glucose testing, including an oral glucose tolerance test, according to locally adopted screening guidelines.
Current research emphasizes earlier and more individualized risk assessment. Studies are evaluating composite prediction tools and machine learning approaches based on clinical characteristics, metabolic and lipid measures, inflammatory indices, metabolites and gut microbiota, and first-trimester placental texture. Reproductive factors such as delayed childbearing, low fertility, and menstrual cycle length, as well as recurrence across pregnancies, are being investigated as risk modifiers. The potential relationship between preconception GLP-1 receptor agonist or tirzepatide exposure and subsequent GDM risk is also an emerging area of study; validation across populations and integration with routine screening remain important challenges.

Gestational Diabetes Risk Factors
Risk assessment is moving earlier, from recurrence and pre-pregnancy metabolic exposure to first-trimester biomarkers. Menstrual-cycle characteristics, parity, body mass index and prior gestational diabetes recur as clinical predictors. GLP-1 receptor agonist or tirzepatide use raises questions about pharmacodynamic carry-over into pregnancy. Microbiota-associated metabolites may complement established risk factors, but prospective validation remains open.

Inflammation and GDM Prediction
Early prediction is shifting toward combined inflammatory, placental and clinical signals rather than glucose testing alone. Periodontal inflammation and markers including MMP9, Tnf and matrix metallopeptidase 8 link maternal inflammation with gestational diabetes. Hematological indices, composite risk tools and placental texture analysis extend prediction to gestational diabetes and hypertensive disorders. These approaches remain observational or require temporal validation before routine use.

Gestational Diabetes Screening and Risk
Risk in older or less fertile women, impaired sleep and inconsistent screening are being considered alongside glucose tolerance testing. Delayed childbearing and low fertility may act independently or jointly with age to increase risk. Sleep duration and quality appear relevant to gestational diabetes subtypes, while hospital audits expose gaps in guideline adherence. The common clinical priority is earlier, more reliable identification of women needing testing.
