Diabetic nephropathy
Diabetic nephropathy, also called diabetic kidney disease, is a diabetes-associated disorder of renal structure and function.
Diabetic nephropathy, also called diabetic kidney disease, is a diabetes-associated disorder of renal structure and function. It is characterized by renal injury that may include albuminuria and a decline in glomerular filtration rate, potentially progressing to kidney failure or end-stage renal failure. Injury to glomerular cells such as podocytes, together with inflammation, oxidative stress, and fibrosis, contributes to impaired filtration. Clinical research has examined markers including the uric acid-to-high-density lipoprotein cholesterol ratio, neutrophil-to-high-density lipoprotein cholesterol ratio, serum lactate, and frailty, although these associations remain largely exploratory.
Current mechanistic studies focus on inflammatory and cellular pathways involved in diabetic renal injury, including macrophage infiltration, podocyte damage, ferroptosis, and JAK/STAT signaling. Experimental work has investigated antiglycation, anti-inflammatory, and anti-fibrotic approaches, as well as modulation of the JAML–FPR2 axis. Kidney biopsy studies are also evaluating whether patients labeled as having diabetic nephropathy instead have non-diabetic kidney disease, highlighting the diagnostic heterogeneity of diabetes-associated renal disease.

Diabetes-Related Kidney Disease
Risk assessment spans uric acid-to-HDL, neutrophil-to-HDL, lactate, frailty, and other clinical markers, with observational and Mendelian-randomization evidence still exploratory. Mechanistic studies converge on inflammation, macrophage infiltration, podocyte injury, antiglycation effects, fibrosis, ferroptosis, and JAK/STAT signaling. Kidney biopsy work is refining diagnosis by challenging nonspecific diabetic labels. Overall, the field remains broad and early, linking risk stratification with targeted protection of renal cells.
