Robot-assisted radical prostatectomy

Robot-assisted radical prostatectomy (RARP) is a minimally invasive surgical treatment for localized or selected high-risk prostate cancer in which a surgeon uses a robotic surgical platform to perform radical prostatectomy.

  • Functional Preservation and Surgical Margins

Anatomical preservation during robot-assisted prostatectomy is being refined to improve continence and erectile recovery without sacrificing cancer control. Nerve-sparing, detrusor-apron, periurethral, hood, and superior pelvic-diaphragm fascia preservation recur as targets. Positive surgical margins and biochemical recurrence remain the oncological checks, with margin aetiology guiding technical refinement. The balance between functional recovery and margins, including in non-nerve-sparing surgery, remains central.

  • Patient-Centered Prostatectomy Care

Robot-assisted prostatectomy care is being assessed beyond operative success, from national uptake to counseling quality and patient-reported recovery. Older-men utilization, decisional regret, and early outcomes recur alongside efforts to evaluate large language models for preoperative advice. Chylous ascites, including sequential lymphatic leaks and octreotide treatment, highlights rare but difficult postoperative morbidity. The set broadens outcome assessment, but does not establish a single clinical direction.

  • Robotic Surgery Outcomes and Learning

Robotic surgery is being benchmarked against open and laparoscopic or thoracoscopic approaches across cancer operations, while surgeon learning is tracked within urology. Perioperative outcomes, complications graded by Clavien-Dindo, console time, caseload, and operative progression are the recurring endpoints. The evidence supports assessment of efficiency and safety rather than a universal advantage for robotics. Single-institution data and procedure heterogeneity leave the size and generalizability of any benefit unresolved.

NEW DIRECTION

Robot-assisted radical prostatectomy becomes a test domain for evaluating artificial-intelligence–generated preoperative counselling

The guideline-based evaluation of five contemporary large language models used robot-assisted radical prostatectomy as the clinical subject for testing AI counselling rather than as an operative technique or outcome. Although the models differed substantially in factual accuracy, information quality, safety, and readability, none met the recommended sixth-grade reading level. This shifts RARP into a role as a benchmark for assessing the clinical communication performance of language models and the need for structured oversight before their use in patient counselling 42645620Aug.

NEW DIRECTION

Robot-assisted radical prostatectomy is treated as a population-level driver of age-related treatment expansion

The nationwide Japanese time-series study of prostate cancer surgery 42606800Aug gives robot-assisted radical prostatectomy a role beyond an individual operation whose technical, functional, or perioperative outcomes are measured: it identifies RARP expansion as part of a system-wide shift in treatment selection, with procedures increasing disproportionately among men aged 75 years and older, including those aged 80 years and older. This reframes RARP as a population-level influence on who receives radical prostatectomy, rather than solely as a surgical modality evaluated within selected patient cohorts.
