  • MASH Fibrosis Treatment

Resmetirom is moving MASH care toward fibrosis-directed treatment within broader metabolic and cardiovascular-kidney risk management. Phase 3 evidence, genotype analyses, and trials toward cirrhosis focus on histologic fibrosis improvement and response heterogeneity. Semaglutide and management of type 2 diabetes and hypertension remain complementary approaches. AI pathology and integrated syndrome models are being used to clarify disease progression, regression, and treatment selection.

  • MASH Treatment Economics

Treatment choices for MASH with significant fibrosis are being framed by cost-effectiveness as well as clinical efficacy. Models compare resmetirom 80 or 100 mg and semaglutide 2.4 mg with standard care, using mortality, effectiveness, and incremental cost-effectiveness ratios. Results depend on drug price, fibrosis benefits, long-term survival, and assumptions about disease progression. LY6D–GRB2–AMPK–SREBP1 signaling adds a mechanistic avenue for targeting hepatic steatosis, but it is not yet linked to the economic comparisons.

  • Metabolic Inflammation Beyond Liver

Resmetirom is being examined as a way to counter metabolic inflammation beyond hepatic steatosis. In MASH models, combining it with C. butyricum targets the gut–liver–brain axis and involves NF-κB, oxidative stress, 5-HT, and microbiome changes. In high-fat diet ApoE-/- mice, it reduces dyslipidemia, atherosclerosis-associated vascular inflammation, and related inflammatory burden. The shared direction is preclinical expansion from liver disease toward systemic cardiometabolic protection, while clinical relevance remains open.
