Proprotein convertase subtilisin/kexin type 9 inhibitors
Proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9 inhibitors, or PCSK9i) are lipid-lowering therapies that target PCSK9, a protein involved in the degradation of the hepatic low-density lipoprotein receptor (LDLR).
Inhibiting PCSK9 preserves LDLR-mediated clearance of LDL cholesterol from the circulation and is therefore used as a therapeutic strategy for hyperlipidemia and arteriosclerotic cardiovascular disease. PCSK9-targeting pharmacotherapy can also reduce lipoprotein(a), although comparative real-world data across available agents remain limited. Inclisiran is a small interfering RNA that targets PCSK9 and has been studied for durable LDL-cholesterol lowering and treatment persistence.
Current research is extending beyond conventional inhibition toward protein–protein interaction inhibitors, computationally identified compounds, and durable genetic approaches such as the investigational liver-directed base-editing therapy VERVE-102. Clinical studies are evaluating PCSK9 inhibitors in settings including ischemic stroke attributable to intracranial atherosclerosis, while the efficacy of adjunctive treatment in that context remains unclear. Additional work examines PCSK9 in vascular aging, macrophage-mediated tumor resistance, and LDL uptake, broadening investigation of the target beyond cholesterol management.

PCSK9 Cardiovascular Therapeutics
PCSK9 research is moving beyond conventional inhibition toward durable LDL-C and Lp(a) reduction through inclisiran, protein–protein interaction inhibitors, and in vivo base editing with VERVE-102. Clinical work emphasizes treatment persistence, cardiovascular risk, and possible benefit after intracranial atherosclerotic stroke. Genetic and proteomic studies continue to clarify how PCSK9, LDLR, and vascular aging shape disease. Macrophage-targeting and natural-product approaches broaden the field, but their therapeutic relevance remains less established.
