p-Tau217
p-Tau217 is tau protein phosphorylated at the 217th amino-acid residue.
p-Tau217 is tau protein phosphorylated at the 217th amino-acid residue. It is studied primarily as a blood-based biomarker of Alzheimer’s disease (AD)-related pathology rather than as a therapeutic drug target. Measurements are commonly performed in plasma, although p-tau217 may also be evaluated alongside cerebrospinal-fluid (CSF) biomarkers and imaging methods such as amyloid positron emission tomography (PET).
Across the provided literature, plasma p-tau217 is investigated in relation to amyloid beta (Aβ), particularly Aβ42, Aβ40, and the Aβ42/Aβ40 ratio; other biomarkers such as p-tau181, glial fibrillary acidic protein (GFAP), and neurofilament light chain (NF-L); and clinical outcomes including mild cognitive impairment, cognitive impairment, cognitive decline, and dementia. Its principal medical role is the assessment of amyloid pathology and the identification or monitoring of individuals within the Alzheimer’s disease continuum, including candidates for anti-amyloid therapy.

Plasma Biomarker Validation
Clinical use is moving toward plasma p-tau217 as a practical marker of amyloid pathology and treatment eligibility. Lumipulse performance is being tested against other platforms, with attention to analytical robustness, dynamic range, and agreement with PET and amyloid status. p-tau181, amyloid beta 42, and longitudinal biomarker relationships provide context for interpretation. Discordant p-tau217 results, including across ethnically diverse memory-clinic cohorts, leave cutoffs and discordance management open.

Cognitive Decline Prediction
Plasma p-tau217 is being evaluated as a prognostic signal for cognitive impairment and dementia. Studies link baseline levels with subsequent cognitive decline and examine how biomarker trajectories relate to subjective cognitive decline and clinical risk factors. Longitudinal data in Down syndrome extend the question beyond typical memory-clinic populations. The work supports risk stratification, but follow-up duration and generalisability remain limiting.

Blood Neurodegeneration Biomarkers
Blood biomarkers are being used to relate neurodegenerative pathology to cognition and clinical symptoms. GFAP, neurofilament light chain, and tau recur across studies using immunoassays and remote capillary sampling. The evidence spans Alzheimer's disease and temporal lobe epilepsy, linking biomarker levels with cognitive or neurological status. Their clinical relevance and the reliability of minimally invasive sampling remain the main open questions.
