Osteoporosis
Osteoporosis is a systemic skeletal disease characterized by reduced bone strength and increased susceptibility to fragility fractures.
Osteoporosis is a systemic skeletal disease characterized by reduced bone strength and increased susceptibility to fragility fractures. Bone strength depends on both bone mineral density and the quality, architecture, and turnover of bone tissue. The condition develops when bone resorption by osteoclasts exceeds bone formation by osteoblasts over time, disrupting normal bone remodeling. Loss of trabecular and cortical bone may occur without symptoms until a fracture develops, commonly involving the vertebrae, hip, wrist, or proximal humerus.
Bone remodeling is regulated by mechanical loading, calcium and vitamin D availability, endocrine signals, and local molecular pathways. Osteoclastogenesis is promoted primarily through receptor activator of NF-κB ligand signaling, whereas osteoblast differentiation and matrix production involve factors including RUNX family transcription factor 2 and osteocalcin. Alkaline phosphatase is a marker associated with osteoblast activity and mineralization. oxidative stress, including reactive oxygen species and changes in Superoxide Dismutase (SOD) activity, may also influence bone-cell function. Osteoporosis is particularly common with advancing age and estrogen deficiency after menopause, although low body mass index, limited physical activity, nutritional deficiencies, certain medications, and other diseases can contribute.
Diagnosis commonly incorporates clinical fracture risk assessment and measurement of bone mineral density, usually by dual-energy X-ray absorptiometry. osteopenia denotes bone density below the young-adult reference range but not low enough to meet the conventional densitometric definition of osteoporosis; regional osteopenia can also occur in localized disorders. Management includes weight-bearing and resistance exercise, fall prevention, adequate calcium and vitamin D intake, and pharmacological treatment when fracture risk is sufficiently high. Antiresorptive therapies include bisphosphonates and denosumab, while anabolic or bone-forming approaches include teriparatide, abaloparatide, and romosozumab. The overall therapeutic objective is to reduce fracture risk while maintaining or improving bone strength.

Bone Loss Prevention and Repair
Bone health research is moving from risk description toward targeted prevention, anabolic delivery and individualized monitoring. Preclinical work tests naringin after orchidectomy and bone-targeted LNP delivery of anti-Sclerostin antibody mRNA, while dietary patterns aim to reduce future loss. Proteomic prediction and telomere-related remodeling measures seek earlier risk stratification. Fracture-fixation complications show that bone quality also matters for repair outcomes.

Biomarkers and Treatment Access
Molecular studies are seeking chronic stress and CKD-linked biomarkers that could improve osteoporosis diagnosis and treatment selection. Bioinformatics, machine learning and multi-omic analyses repeatedly prioritize circulating proteins and shared disease pathways. In parallel, budget-impact work evaluates biosimilars and six-monthly denosumab as ways to broaden access. Clinical utility, validation and affordability remain open questions.

Bone Density Screening
CT-measured trabecular attenuation at the L1 vertebra is being assessed as a practical surrogate for bone mineral density when DXA is unavailable or difficult to deploy. Spine-surgery practice is moving toward routine osteoporosis screening, with dual-energy X-ray absorptiometry still the reference test. Hemodialysis patients and fracture-risk populations remain important targets. The recurring question is whether imaging and screening results change treatment decisions.
