  • NfL and GFAP Biomarkers

Blood and CSF NfL and GFAP are being tested as markers of neuronal and glial injury across diverse neurological presentations. Single Molecule Array measurements recur in hepatic encephalopathy, myasthenia gravis, multiple sclerosis, epilepsy, autoimmune encephalitis, transient ischemic attack, and paediatric psychiatry. The work is moving toward minimally invasive diagnosis and prognosis, but disease specificity remains limited. How these markers should be interpreted across compartments and disorders remains open.

  • NfL in Multiple Sclerosis

Clinical NfL testing in multiple sclerosis is moving toward standardized, age-aware interpretation rather than a raw concentration alone. Roche, Fujirebio and Siemens assays are being compared, while age- and eGFR-adjusted reference ranges address major sources of variation. Plasma NfL adds prognostic information beyond MRI for disability and radiological progression, with EDSS and biological-age gaps as recurring outcomes. Its role as a surrogate in GM2 gangliosidosis broadens validation, but prospective treatment-response evidence remains limited.

  • Alzheimer Biomarkers in Down Syndrome

Plasma NfL and Alzheimer biomarkers are being followed longitudinally to connect biological change with cognitive decline in Down syndrome. Proteomic and cognitive trajectories support earlier detection across the Alzheimer spectrum, but the timing and screening value of individual markers remain unsettled. Prion disease cohorts test whether Alzheimer plasma signatures retain specificity outside the target disorder. The field is therefore moving toward multimarker, longitudinal interpretation rather than a single screening analyte.

  • NfL in Neuroaxonal Injury

Plasma NfL is being positioned as a marker of neuroaxonal injury across treatment-related, metabolic and motor-neuron disease. Longitudinal measurements paired with patient-reported outcomes aim to track vincristine-associated peripheral neuropathy, while diabetes studies relate NfL to neuropathy, age and renal function. In ALS, NfL outperforms inflammatory, metabolic and blood–brain barrier markers for prognosis, supporting its use alongside rather than instead of clinical measures. Whether it can detect early or treatment-responsive peripheral nerve damage remains unresolved.

PARADIGM SHIFT

Plasma NfL results cannot be treated as interchangeable across automated assay platforms

The multiple-sclerosis cohorts comparing Roche Elecsys with Fujirebio Lumipulse and Siemens Atellica with Lumipulse found that, although measurements were correlated, raw values showed poor or only moderate agreement and proportional bias; alignment improved only after conversion to a common reference scale 42640285Aug 42378934Jun. This overturns the assumption that an NfL concentration has the same clinical meaning regardless of platform and makes assay-specific harmonization or conversion necessary before results can be compared or interpreted using shared thresholds.

NEW DIRECTION

NfL is used to construct a biological-age measure in multiple sclerosis

The neurofilament light chain-derived age-gap study in multiple sclerosis and controls assigned NfL a role beyond measuring current axonal injury or predicting disease activity: it used age-adjusted NfL values to estimate biological age and relate the difference from chronological age to disease characteristics 42802281Sep. This introduces NfL as an index of accelerated biological aging, rather than solely as a disease or treatment biomarker.
