Microglia
Microglia are immune cells of the central nervous system that monitor neural tissue and respond to injury, infection, and other pathological signals.
Microglia are immune cells of the central nervous system that monitor neural tissue and respond to injury, infection, and other pathological signals. Their activation can alter inflammatory signaling, phagocytosis, apoptosis, and tissue repair. Current research links microglial responses to neuroinflammation in neuropathic pain, ischemic stroke, intracerebral hemorrhage, Alzheimer’s disease, and postoperative delirium, with associated changes examined in the brain, hippocampus, and spinal dorsal horn. Microglial signaling is studied alongside astrocytes, neurons, the blood–brain barrier, reactive oxygen species, interleukin-1β (IL-1β), and the NLRP3 inflammasome.
Microglia are also being investigated as therapeutic targets and biomarkers. Experimental approaches aim to regulate their functional states through pathways involving Nrf2/NLRP3, CXCL16, mitophagy, ADA-dependent adenosine metabolism, and staged control of oxidative stress and inflammation; reported delivery systems include ROS-responsive hydrogels and nanozymes. Transcriptomic analyses of microglial phagocytosis and live human brain multi-omics have been used to identify disease-associated molecular signatures, while hematopoietic stem cell transplantation research examines the engraftment of stem-cell progeny as microglia-like cells. These findings support microglial modulation as a potential strategy for limiting tissue damage and promoting neurorepair, although the mechanisms and diagnostic value of distinct microglial states remain under investigation.

Neuroinflammation and Neurorepair
Oxidative stress and inflammatory microglial states are being targeted to limit brain injury and promote repair. Donepezil nanosystems aim to activate mitophagy, while ROS-responsive hydrogels stage immunomodulation after hemorrhage. Que-Mn nanozymes and recombinant CXCL16 address neuroinflammation in neuralgia and ischemic stroke. The recurring strategy is multimodal control of microglial phenotype, apoptosis, and redox injury across neurological disease.

Microglial Activation Pathways
Microglial activation is linked to neuropathic pain, pelvic pain, and Alzheimer’s disease through inflammatory and phagocytic mechanisms. Daphnetin acts through the Nrf2/NLRP3 inflammasome pathway, whereas sacral nerve modulation targets the spinal immune microenvironment. Transcriptomic phagocytosis signatures are being pursued as Alzheimer’s diagnostic markers. The field is moving toward mechanism-based modulation and molecular stratification, but the pain and dementia applications remain distinct.

Others — Microglial Models and Injury
Microglial identity, particle cytotoxicity, and postoperative delirium are examined through complementary cellular and human-tissue approaches. Hematopoietic stem-cell-derived microglia-like cells are being characterized in nonhuman primates, while amine-modified silica particles are assessed for toxicity. Live human brain multi-omics instead maps neuroinflammation after surgery. The studies share microglial characterization but do not establish a common therapeutic direction.
