  • Kidney Protection in CKD

Finerenone is being tested across diabetic and non-diabetic CKD, including IgA nephropathy, with eGFR and proteinuria as recurring endpoints. Real-world studies are moving toward practical use alongside SGLT2 inhibitors and in patients with preserved kidney function. Hyperkalemia and serum potassium remain the main safety constraints, while ischemic stroke and long-term kidney outcomes are also being assessed. Mechanistic work extends the focus to fibrosis, including peritoneal fibrosis and restored autophagy flux.

  • Mineralocorticoid Blockade in Heart Failure

Mineralocorticoid receptor blockade is moving into heart failure with mildly reduced or preserved ejection fraction, where finerenone is being compared with spironolactone. SGLT2 inhibitor use, transportability of trial effects, and 12-month clinical events frame the main clinical questions. The evidence remains largely comparative and observational rather than definitive for superiority. Prescription patterns suggest broader cardiovascular-kidney use, but optimal patient selection and outcomes remain open.

  • Cardiac Microvascular Injury

Diabetic cardiomyopathy is being linked to cardiac microvascular injury, endothelial-cell damage, and neutrophil extracellular trap formation. Finerenone is being evaluated preclinically for improving cardiac dysfunction while modulating AKT-related signaling and damage-associated molecular patterns. The evidence is early and comes mainly from animal models, so effects on clinical heart failure endpoints remain unknown. A separate bosentan combination study broadens the work toward pharmacologic protection from cardiac dysfunction.

NEW DIRECTION

Finerenone is presented as an autophagy-flux restorative treatment for peritoneal fibrosis

The peritoneal-fibrosis study in peritoneal-dialysis mesothelial cells assigns finerenone a role beyond the set’s usual cardiorenal treatment and mineralocorticoid-receptor framework: it attenuates fibrosis by restoring autophagy flux. This makes autophagy regulation in injured peritoneal mesothelium, rather than cardiorenal risk reduction, the drug’s proposed therapeutic function 42209198May.
