  • Stroke Prevention in Atrial Fibrillation

Anticoagulation is moving toward individualized stroke prevention after ischemic stroke, endovascular therapy, myocardial infarction, and in intermediate-risk atrial fibrillation. Timing, adherence, and patient preferences recur as determinants of benefit. Direct oral anticoagulants are compared with warfarin in cancer and routine care. Implementation studies seek better uptake and outcomes, but the optimal timing and intensity remain unsettled.

  • Restarting Anticoagulation after Brain Bleeding

Restarting direct oral anticoagulants after traumatic subdural or intracranial hemorrhage is being weighed against recurrent bleeding and embolic stroke. Atrial fibrillation, older age, and gastrointestinal bleeding complicate risk assessment. Observational data describe resumption patterns, while randomized work is testing safer timing. The main uncertainty is when protection should resume without provoking rebleeding.

  • Others — Drug Interactions and Lung Injury

Calcineurin inhibitors such as cyclosporine and tacrolimus raise drug-interaction concerns when combined with apixaban or edoxaban in transplant recipients. CYP3A4 activity and anticoagulant exposure are central to dosing decisions. A separate study examines whether oral anticoagulants are associated with acute interstitial lung disease. These topics share anticoagulant safety but form no unified biological or clinical question.
