Adjuvant chemotherapy
Adjuvant chemotherapy is systemic anticancer treatment administered after definitive local therapy, most commonly surgery, to reduce the risk of recurrence from residual microscopic disease.
Adjuvant chemotherapy is systemic anticancer treatment administered after definitive local therapy, most commonly surgery, to reduce the risk of recurrence from residual microscopic disease. Cytotoxic agents such as oxaliplatin, gemcitabine, and fluoropyrimidines act primarily by disrupting DNA replication or cell division, thereby affecting rapidly proliferating cancer cells. Its use is disease- and risk-specific, with treatment decisions incorporating stage, histology, lymph-node involvement, resection findings, predicted recurrence risk, and patient fitness.
Adjuvant chemotherapy is used or evaluated across several malignancies, including breast, gastric, colorectal, rectal, pancreatic, and cervical cancer. In early breast cancer, genomic testing such as the 21-gene recurrence score can help guide chemotherapy decisions in hormone receptor-positive, human epidermal growth factor receptor 2-negative disease, while the benefit remains debated for some small, node-negative triple-negative tumours. In gastrointestinal cancers, studies assess its contribution to disease-free survival and overall survival after resection, including in stage II–III gastric cancer and after rectal cancer treatment involving total neoadjuvant therapy. It may also be combined with radiation therapy or concurrent chemoradiotherapy in selected settings. Treatment completion can be limited by postoperative complications, age, frailty, and symptoms, making toxicity, functional status, and quality of life important considerations alongside recurrence reduction.

Gastrointestinal Adjuvant Treatment
Adjuvant treatment decisions in gastrointestinal cancer are being shaped by stage, postoperative fitness and the likelihood of completing chemotherapy. Gastric and rectal cancer analyses assess survival benefit using hazard ratios and disease-free survival, including T3N0 disease and FOLFIRINOX-based total neoadjuvant therapy. Frailty screening with G8 and lymphocyte counts highlights early discontinuation after complications, especially in older adults. The unresolved question is how to preserve oncological benefit while limiting treatment burden.

Genomic Selection in Breast Cancer
Treatment selection is increasingly combining genomic risk with clinicopathological features in early breast cancer. The 21-gene recurrence score is used to refine chemotherapy decisions, while small node-negative triple-negative tumours remain a question of absolute benefit. Patient-reported outcomes recur through QLQ-C30 symptoms and functional impairment during treatment. The field is balancing recurrence reduction against toxicity and quality of life.

Multimodality Therapy in Gastrointestinal Cancer
Multimodality treatment is being tested by comparing chemoradiotherapy alone with regimens that add adjuvant chemotherapy across gastrointestinal cancers. Outcomes centre on overall and disease-free survival, mortality and treatment benefit in stage III or resected disease. The lymph node ratio is used to stratify individualized benefit in gastric cancer, while pancreatic trials examine the contribution of radiotherapy after chemotherapy. Results remain disease-specific, leaving the optimal sequencing and selection of patients for combined treatment open.
