Ranibizumab

Overview

Ranibizumab is an antibody fragment directed at vascular endothelial growth factor A (VEGF-A), given by intravitreal injection for retinal disease driven by pathological neovascularization and vascular leakage. It is a Fab rather than a whole antibody, and the missing Fc region is the design: without it the molecule is smaller, penetrates the retina readily, and is cleared from the systemic circulation quickly once it leaves the eye, limiting suppression of VEGF elsewhere in the body. It shares a parent antibody with bevacizumab, which is used off-label for the same indications at a fraction of the cost — a comparison settled by head-to-head trials showing similar visual outcomes, and one that continues to shape practice worldwide.

Blocking VEGF-A removes the principal driver of both new vessel growth and the vascular permeability that produces macular oedema. That accounts for its use in neovascular age-related macular degeneration, myopic choroidal neovascularization, diabetic macular oedema, diabetic retinopathy and macular oedema following retinal vein occlusion. It is not standard therapy in central serous chorioretinopathy, where the underlying process is choroidal rather than VEGF-driven, unless secondary choroidal neovascularization has developed.

The practical constraints are the injections themselves. Effect duration is short, so treatment began as monthly dosing and has moved to treat-and-extend regimens that individualize the interval, with longer-acting agents such as aflibercept and faricimab and a refillable port delivery implant developed to reduce the burden. Each injection carries a small risk of endophthalmitis, retinal detachment and intraocular pressure rise, and biosimilars have entered the market as patents expired. Current research concerns variability in response between patients, long-term outcomes over years of repeated treatment, and intraocular pharmacokinetics — how long a given dose actually suppresses VEGF within the eye.

Recent Publications Summary

Recent publications on ranibizumab focused on its use across several retinal and ocular conditions, including myopic choroidal neovascularization, diabetic macular oedema, neovascular age-related macular degeneration, retinal vein occlusion, central serous chorioretinopathy, and a non-retinal anterior segment complication in Fuch’s heterochromic iridocyclitis. In a 10-year retrospective series of myopic CNV, intravitreal ranibizumab was associated with an initial improvement in best-corrected visual acuity, with mean BCVA improving from 0.49 logMAR at baseline to 0.29 logMAR at 1 year, and the study also reported long-term injection burden and changes in chorioretinal patchy atrophy 42175486May. Other studies evaluated ranibizumab within broader anti-VEGF treatment strategies or comparative settings. A computational pharmacokinetic model simulated intravitreal ranibizumab distribution in the human eye, estimating macular drug concentration and intravitreal half-life under different vitreous and elimination scenarios; predicted half-lives ranged from 2.7 to 4.4 days and were consistent with reported human data 42096431May. In a nationwide Japanese claims analysis, ranibizumab was one of the anti-VEGF agents tracked from 2017 to 2022, alongside aflibercept, brolucizumab, and faricimab, to assess injection trends, regional variation, seasonality, and the impact of COVID-19 on intravitreal injection use 42096458May. A separate study in central serous chorioretinopathy investigated whether indocyanine green angiography-based texture analysis could predict response to anti-VEGF treatment, with ranibizumab included among the therapies under consideration 42192607May.

Ranibizumab was also used as a comparator or adjunct in more specialized clinical reports. In the STAR trial analysis of neovascular age-related macular degeneration, pro re nata ranibizumab served as the anti-VEGF backbone against which stereotactic radiotherapy was assessed, with the trial noting reduced retreatment needs but worse BCVA in later years in the radiotherapy arm 42449377Jul.
