CD3+

CD3+ refers to cells that express the cluster of differentiation 3 (CD3) glycoprotein complex, a surface antigen expressed on the majority of T lymphocytes and a critical component of the T cell receptor (TCR) signaling machinery.

  • Bispecific T-Cell Engagers

CD3-directed engagers are expanding from BCMA targeting toward multispecific and affinity-tuned designs for hematologic and solid tumors. DNA hypomethylating agents may preserve T-cell stemness and improve bispecific activity, while local EpCAM, IL-12 and GM-CSF mRNA delivery seeks systemic control from intratumoral treatment. FAP×CD3 and TCR-mimic constructs extend targeting to solid-tumor antigens, with evidence mainly from preclinical or ex vivo models. Clinical translation, dosing and the durability of tumor control remain open.

  • Myeloma Bispecific Therapy

CD3 bispecifics are moving deeper into relapsed or refractory myeloma treatment, with teclistamab and talquetamab also being tested in combinations such as talquetamab-daratumumab. Cytokine release syndrome, infections and prolonged B-cell depletion recur across treatment experience. GPRC5D-directed talquetamab adds distinctive toxicities that require active recognition and management, while pharmacovigilance is broadening safety signals beyond trials. Earlier-line use and safer sequencing remain unresolved.

  • B-ALL Relapse Prevention

Blinatumomab is being integrated around allogeneic transplantation to redirect donor lymphocytes against B-ALL and strengthen graft-versus-leukemia activity. Pretransplant exposure is associated with lower relapse and better disease-free survival in adults undergoing transplant. Periallotransplant BiTE use aims to preserve this antileukemic benefit while limiting treatment burden and immune complications. The balance with graft-versus-host disease, including chronic disease, and the need for prospective confirmation remain open.

PARADIGM SHIFT

CD3 redirection can control tumors through immune and stromal remodeling, not only direct killing of antigen-bearing tumor cells

The FAP×CD3 engager OMTX305 in patient-derived lung and ovarian cancer tumor slices and intratumoral EpCAM BITE combined with IL-12 and GM-CSF mRNA-LNPs in bilateral human xenograft tumors both depart from the assumption that CD3 engagement works principally by directly lysing targeted tumor cells. OMTX305 instead eliminated FAP-expressing fibroblasts and triggered bystander killing of adjacent tumor cells, while the EpCAM strategy expanded T-cell activity in both injected and distant untreated tumors and induced HLA class II programs that could make cancer cells antigen-presenting. Together, these findings shift the therapeutic frame toward CD3-driven remodeling of the tumor microenvironment and propagation of antitumor immunity beyond the initially engaged target cells. 42726846Sep 42696537Sep

NEW DIRECTION

CD3 expression is proposed as a genetically mediated immune regulator of primary aldosteronism

The FES eQTL study in primary aldosteronism and TD CD8br immune cells assigns CD3 a role distinct from its use as the T-cell-engaging arm of anticancer bispecifics. Using Mendelian randomization, colocalization, and mediation analyses, it associated FES expression with CD3 expression on TD CD8br cells and linked increased CD3 expression with reduced risk of primary aldosteronism. This places CD3 within a putative immune-genetic pathway influencing endocrine disease risk rather than solely within therapeutic T-cell redirection. 42115443May
