CD20
CD20 is a cell-surface protein expressed on B cells and serves as an important therapeutic target in B-cell malignancies and immune-mediated disease.
CD20 is a cell-surface protein expressed on B cells and serves as an important therapeutic target in B-cell malignancies and immune-mediated disease. Antibodies directed against CD20, including rituximab, obinutuzumab, and ocrelizumab, are used to induce depletion of CD20-positive B cells. This approach has been applied in follicular lymphoma, mantle cell lymphoma, hairy cell leukemia, multiple sclerosis, and autoimmune diseases such as primary membranous nephropathy.
CD20 is also targeted by bispecific antibodies that connect CD20-positive B cells with CD3-positive T cells, thereby redirecting T-cell activity toward the target cell. Epcoritamab, glofitamab, and mosunetuzumab exemplify this strategy, with studies reporting activity in relapsed or refractory lymphomas and investigation of deeper responses, including measurable residual disease negativity. Current research is extending CD20-directed therapy through combinations such as mosunetuzumab with polatuzumab vedotin, examination of CD20 organization in cholesterol-rich membrane domains, and engineered expression of CD20 on otherwise unsuitable solid-tumor targets.

B-Cell Depletion Immunotherapy
CD20-directed B-cell depletion is moving toward T-cell redirection and deeper response assessment in B-cell malignancies. Glofitamab and epcoritamab studies emphasize CD3 engagement, progression-free survival, and MRD negativity by PBMCs or ctDNA. CD4+ T-cell reserve may help predict bispecific-antibody benefit, while rituximab, obinutuzumab, BTK inhibition, and transplant-based approaches remain complementary strategies. Durability, immune competence, and prevention of EBV-associated lymphomagenesis remain unresolved.

Multiple Sclerosis B-Cell Therapy
B-cell depletion in multiple sclerosis is being tied to biological monitoring rather than treatment choice alone. Ocrelizumab studies pair serum neurofilament light chain with IL-6 and IL-1β inflammatory or antiviral signatures to assess activity and outcomes. The recurring question is whether these markers predict response or residual disease after therapy. Evidence remains cross-sectional or retrospective, leaving prospective validation and clinically useful thresholds open.

Antibody-Directed Solid Tumor Killing
Rituximab is being repurposed as a potential solid-tumor immunotherapy through ectopic CD20 expression. Complement-dependent cytotoxicity and macrophage-mediated killing recur as key effector mechanisms. Spatial organization of antibody–receptor complexes in cholesterol-rich membrane domains offers a mechanistic explanation for differences in activity. The approach remains an early proof of concept, with tumour selectivity and effective immune recruitment still open.

CD20 can be artificially installed on solid tumors to redirect an established anti-CD20 antibody against them
The cancer-cell-selective CD20-expression study in colorectal cancer cells, spheroids, organoids, and a humanized mouse model assumed CD20 was a native antigen used to target CD20-bearing tumors with rituximab. Instead, it selectively introduced CD20 into otherwise non-indicated tumors using an NF-κB-controlled gene-delivery system, after which rituximab recruited NK-cell-mediated antibody-dependent cellular cytotoxicity and macrophage-mediated antibody-dependent cellular phagocytosis. CD20 therefore becomes an engineerable therapeutic tag for extending anti-CD20 treatment to solid tumors rather than only a naturally occurring tumor antigen 42671316Aug.
