Bispecific antibodies
Bispecific antibodies are engineered antibody therapeutics designed to bind two distinct molecular targets or epitopes.
Bispecific antibodies are engineered antibody therapeutics designed to bind two distinct molecular targets or epitopes. By bringing two interacting components into proximity, they can redirect immune cells, block complementary biological pathways, or enhance neutralization of infectious agents. In cancer immunotherapy, antibodies targeting a tumor-associated antigen and CD3+ T cells can recruit T cells to tumor cells, while T-cell receptor (TCR)-mimic bispecific antibodies can recognize selected peptide–HLA complexes and redirect T-cell cytotoxicity. These approaches are being investigated in hematologic and solid cancers, including relapsed/refractory multiple myeloma, and in combination or sequence with chimeric antigen receptor T-cell therapy.
Other bispecific designs target two conserved viral epitopes to broaden neutralization, including preclinical constructs directed against H7 avian influenza and severe fever with thrombocytopenia syndrome virus. Bispecific antibodies are also being developed to disrupt tumor–stroma interactions, such as those involving FAP and TREM2, and to combine β-nerve growth factor and TNFR2 binding. Their development therefore spans cancer immunotherapy, antiviral treatment, and modulation of disease-associated cellular niches; characterization of anti-drug antibodies and domain-specific neutralizing antibodies is relevant to their clinical evaluation.

Treatment Sequencing in Myeloma
Relapsed/refractory multiple myeloma is moving toward deliberate sequencing of CAR-T therapy and bispecific antibodies rather than treating them as isolated options. Comparative studies examine single-modality and sequential strategies, with long-term survival and treatment response as key endpoints. CNS involvement remains a high-risk clinical context requiring separate risk and response assessment. The optimal order, durability, and patient selection remain unresolved.

Neutralizing Antibody Development
Antiviral bispecific antibodies are being pushed toward broad, potent neutralization across viral variants and pathogens. Conserved hemagglutinin targeting protects against H7 avian influenza, while another construct provides complete preclinical protection against SFTSV. Neutralization assays are also being developed to characterize antibodies against a β-NGF/TNFR2 therapeutic. The field remains largely preclinical, with breadth, potency, and assay performance still to establish.

Cancer T-Cell Engagement
T-cell redirection in cancer is shifting toward precise recognition of peptide-HLA targets and stronger persistence in hostile tumor niches. TCR-mimic antibodies are being engineered to bind selected peptide-HLA complexes, while DNA hypomethylating agents preserve T-cell stemness and potentiate CD3-bispecific activity. Single-cell and spatial multi-omics point to a tumor-stroma interface involving tumor-associated macrophages in multidrug resistance. Linking these niches to response and progression remains an open clinical question.

Dual-epitope bispecific antibodies can overcome the limited breadth and durability of conventional single-antibody antiviral protection
The SFTSV nanobody agent N2D2-Fc-N2C1 and the H7 avian-influenza agent BsAb-H7 each showed, in independent viral systems, that combining antibodies against distinct epitopes produces protection beyond that of the parental or conventional single-antibody approach: the SFTSV bispecific had greater potency and protected mice and ferrets even after infection, while BsAb-H7 neutralized divergent H7 subtypes and protected against heterologous infection despite the escape risk associated with HA-head antibodies. This shifts the emphasis from potent neutralization of one viral target to coordinated engagement of conserved, non-overlapping epitopes as a means of achieving breadth and therapeutic durability 42814544Sep 42617142Aug.
Bispecific antibodies can be used to dismantle a spatial tumor–stroma resistance niche rather than chiefly to neutralize pathogens or redirect cytotoxic T cells
The FAP/TREM2 bispecific antibody developed for multidrug-resistant melanoma targets the interacting fibroblast and macrophage components of a recurrent tumor–stroma interface, disrupting the CXCL14–TREM2-associated niche and restoring treatment sensitivity in patient-derived xenografts. This assigns bispecific antibodies a distinct role as spatial microenvironment remodelers that reverse established therapeutic resistance, rather than as direct antiviral neutralizers or tumor-cell-directed T-cell engagers 42748914Sep.
