B-cell Maturation Antigen
B-cell maturation antigen (BCMA) is a protein that serves as a therapeutic target on malignant plasma cells, particularly in multiple myeloma.
B-cell maturation antigen (BCMA) is a protein that serves as a therapeutic target on malignant plasma cells, particularly in multiple myeloma. Its expression on these cells has supported the development of several BCMA-directed approaches, including antibody–drug conjugates such as belantamab mafodotin, bispecific antibodies such as teclistamab, and autologous or allogeneic CAR-T-cell therapies. BCMA is also being investigated in CAR-NK-cell platforms and in treatment strategies for relapsed or refractory AL amyloidosis.
BCMA-directed therapies use different molecular or cellular mechanisms to recognize and eliminate target cells. Teclistamab engages BCMA and CD3 to involve T-lymphocytes, whereas CAR-T and CAR-NK products use engineered immune cells to produce antitumor activity. Ongoing work addresses response, treatment sequencing, persistence, access, and adverse events such as cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome. Epitope engagement and BCMA mutations may influence the relative activity of different agents, while soluble BCMA and serum M-protein are being evaluated as disease-related pharmacodynamic markers.

BCMA-Directed Treatment in Myeloma
BCMA-directed treatment is broadening from CAR T cells to bispecific antibodies and antibody-drug conjugates in relapsed/refractory myeloma. Ciltacabtagene autoleucel and anito-cel studies emphasize activity, while teclistamab and belantamab analyses add real-world safety and pharmacokinetic evidence. Cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome recur as key risks, including in frail patients. Epitope engagement, BCMA mutations, soluble BCMA, and serum M-protein are emerging as determinants of response and treatment monitoring.

Cellular Immunotherapy for Myeloma
BCMA cellular immunotherapy is expanding beyond conventional autologous CAR-T toward triple-knockout allogeneic CAR T cells and CAR-NK cells. Early studies prioritize feasibility, cytotoxicity, and durable responses in heavily treated disease. Durable drug-free remission after BCMA CAR T-cell therapy also broadens interest beyond myeloma to antibody-mediated disorders. CTLA4 and LAG3 polymorphisms may help explain differences in response, but clinical evidence remains early.

BCMA-Targeted Outcomes
BCMA-targeted therapies are being evaluated across relapsed or refractory multiple myeloma and AL amyloidosis, with bispecific antibodies among the central approaches. Progression-free survival and median time to next treatment remain the principal outcome measures. The evidence is largely descriptive, drawing on the French EMMY real-world cohort and pooled analyses rather than direct comparative trials. The field is moving toward broader disease application, while comparative effectiveness and long-term safety remain unresolved.
