Nirsevimab

Nirsevimab is a long-acting monoclonal antibody used for the prevention of respiratory syncytial virus (RSV) lower respiratory tract disease in infants.

  • Infant RSV Prevention

Infant RSV prevention is being compared across nirsevimab, maternal RSVpreF vaccination, and combined strategies. Real-world studies focus on protection duration and reductions in RSV-related hospitalization among high-risk infants. Economic analyses increasingly weigh population-wide immunization against targeted use. Breakthrough RSV-B infections raise a continuing concern about nirsevimab resistance.

  • RSV Immunization Programs

RSV immunization programs are being judged by coverage, uptake, safety, and cost-effectiveness. Nirsevimab and maternal RSVpreF vaccination are modelled as separate or hybrid approaches for preventing infant disease. Implementation studies identify newborn uptake and pediatrician knowledge as practical constraints. Post-licensure surveillance is expanding safety assessment to children aged 0–24 months.

  • RSV Immunization Acceptance

RSV immunization decisions center on acceptance, product choice, timing, and post-licensure safety. Surveys examine pregnant women’s preferences for maternal vaccination, postpartum acceptance of nirsevimab, and pediatricians’ knowledge of available products. Policy modelling compares seasonal with year-round infant immunization to reduce hospitalizations. Early program reports and Canadian surveillance are adding practical uptake and safety evidence.

  • RSV Prevention Outcomes

RSV prevention is being measured through outcomes beyond infection alone, especially hospitalization and acute otitis media. Interrupted time-series data associate nirsevimab with fewer acute otitis media infections in infants. Multicentre studies report reductions in pediatric RSV hospitalizations after regional immunization, but the size of benefit varies between settings. Heterogeneous effects remain an important question for program evaluation.

NEW DIRECTION

Nirsevimab becomes an object of evolutionary resistance surveillance, not only an infant-prevention intervention

The French multicentre study of RSV-B breakthrough infections examines nirsevimab escape using genotypic and phenotypic methods rather than evaluating nirsevimab primarily as an infant-prevention product. It identifies breakthrough RSV-B infection and possible resistance emergence as the central problem, extending nirsevimab’s role to the surveillance of antibody escape and the durability of its conserved prefusion-F target 42229496Jun.
