Blinatumomab
Blinatumomab is a bispecific T-cell engager (BiTE) that binds CD19 on B cells and CD3 on T cells, bringing the cells together to promote T-cell activation against CD19-positive leukemia cells.
Blinatumomab is a bispecific T-cell engager (BiTE) that binds CD19 on B cells and CD3 on T cells, bringing the cells together to promote T-cell activation against CD19-positive leukemia cells. Its principal therapeutic role is in B-cell acute lymphoblastic leukemia (B-ALL), where it has demonstrated activity in relapsed or refractory disease and measurable residual disease–positive disease. Current studies are extending its use into frontline treatment, consolidation, chemotherapy-replacement strategies in pediatric patients, and treatment before allogeneic hematopoietic stem cell transplantation, including combinations with ponatinib in Philadelphia chromosome–positive disease.
Blinatumomab’s expanding use is accompanied by clinically important immune effects. Treatment has been associated with immunoglobulin depletion, with recovery relevant to infection-prevention strategies, and prior exposure may influence subsequent chimeric antigen receptor T-cell apheresis. Neurotoxicity is an important limitation and can rarely be severe, including reported diffuse cerebral edema. Its activity and mechanism also make it a subject of quantitative studies of ternary complex formation and T-cell activation.

B-Cell Acute Lymphoblastic Leukemia
Blinatumomab is moving from relapsed disease toward frontline chemotherapy replacement, consolidation, and pretransplant treatment in B-cell ALL. Studies test it with ponatinib or imatinib in Ph-positive disease and as a less intensive strategy for children, infants, and high-risk Ph-negative patients. Reduced relapse and improved disease-free survival support pretransplant use, while effects on immunoglobulin recovery and CAR T apheresis remain clinically relevant. Neurotoxicity, including rare catastrophic cerebral edema, remains an important limitation.
Blinatumomab-based therapy can replace chemotherapy rather than merely supplement it in frontline acute lymphoblastic leukemia
The pediatric high-risk B-cell acute lymphoblastic leukemia trial and the adult Philadelphia-positive acute lymphoblastic leukemia trial both challenge the assumption that blinatumomab must be added to conventional chemotherapy: the pediatric study directly tested two blinatumomab cycles against two chemotherapy cycles after consolidation, while the adult study compared ponatinib plus blinatumomab with imatinib plus chemotherapy and found superior 24-month event-free survival with the blinatumomab regimen (83.0% estimated 4-year event-free survival in the pediatric blinatumomab arm; 86% vs. 67% 24-month event-free survival in adults). Together, these independently randomized studies shift the therapeutic direction toward chemotherapy-sparing frontline regimens in distinct pediatric and adult ALL settings 42748428Sep 42874102Oct.
